Liability Waiver & Release of Claims
Briere’s Goalie Schools & Goal Scoring Camps

PLEASE READ CAREFULLY.
This document affects your legal rights. By agreeing with this waiver, you acknowledge and agree to the terms listed below for yourself and/or your child (“Participant”).
1. Assumption of Risk
I understand that ice hockey, skating, training, and athletic activity involve inherent risks of injury, including but not limited to:

Slips, falls, and collisions
-
Contact with other players, objects, or equipment
-
Muscle strains, sprains, fractures, or concussions
-
Cold-related injuries
-
Accidents occurring on or off the ice
I understand these risks and voluntarily choose to allow the Participant to take part in camps, clinics, events, and related activities offered by Briere’s Goalie Schools & Goal Scoring Camps (“the Organization”).
I acknowledge that participation is voluntary, and the Participant may stop at any time.
2. Release of Liability
In consideration for the Participant being allowed to participate, I hereby release, waive, and forever discharge:
Briere’s Goalie Schools & Goal Scoring Camps, its owners, coaches, staff, employees, contractors, volunteers, facility partners, and all affiliated organizations
from any and all claims, liabilities, demands, or causes of action arising from:

Injury
-
Property damage
-
Illness
-
Accidents
-
Death
whether caused by negligence (excluding gross negligence or intentional misconduct), accidents, or the actions of others.
This release applies to activities occurring:

On the ice
-
In the rink facility
-
During off-ice training
-
In locker rooms
-
During travel between program locations (if applicable)
3. Medical Authorization
I authorize the Organization to obtain emergency medical treatment for the Participant if necessary. I understand that reasonable attempts will be made to contact me first unless immediate care is required.
I agree to be financially responsible for all medical treatment, transportation, or emergency services.
4. Health & Safety Responsibilities
I certify that the Participant:

Is in proper physical condition to safely participate in hockey activities

Has no medical conditions that would increase risk beyond what is inherent to the sport

Will wear necessary and approved protective equipment

Will follow all instructions from coaches and staff
If the Participant has allergies, medical needs, or limitations, I agree to disclose them during registration.
5. Conduct & Compliance
I acknowledge that the Participant must:

Demonstrate respect and sportsmanship
-
Follow all rules of the Organization and the facility
-
Use equipment responsibly
-
Act in a manner that does not endanger themselves or others
The Organization reserves the right to remove any participant (with no refund) for unsafe, inappropriate, or disruptive conduct.
6. Personal Belongings
I understand that the Organization is not responsible for:

Lost items
-
Damaged equipment
-
Theft or misplacement of belongings
Participants should label and secure personal items.
7. Media & Photography Release
I grant permission for the Participant to be photographed or recorded during activities. Such media may be used for promotional, educational, or marketing purposes by the Organization.
I may opt out by providing a written request before the start of the program.
8. COVID-19 / Communicable Disease Notice
I understand the risk of exposure to viruses or illnesses in group athletic settings.
By participating, I assume all associated risks and release the Organization from claims related to communicable disease exposure.
9. Binding Agreement
This waiver is binding upon:

The Participant
-
Parents/guardians
-
Family members
-
Heirs
-
Legal representatives
If any portion is deemed invalid, the remainder shall continue in full effect.
11. Acknowledgment
